RESUMO

The past decade experienced a stunning change in the incidence and fatality rates of pesticide poisonings in China. A study on representative case records of pesticide poisoning in a chinese province is presented here. A three-fold increase in poisoning cases due to a lack of preventive measures was reported at the beginning of the 1980s, under the rural economic reformation. Occupational intoxication incidence was swiftly reduced within a couple of years, and further stepped down as the adequate means were taken. On the contrary, the increase in non-occupational cases declined very slowly. The fatality rate of occupational poisoning cases was kept low (around few per thousand), while the non-occupational remarkably declined from 30% to 11% thanks to the efforts of health and medical care centres. The number and fatality rate of non-occupational cases thus came to play a more and more important role. Such trend is worth our attention.

To confirm the main etiological antigen of farmer's lung in Dafeng County, Jiangsu Province, People's Republic of China, precipitins in serum from 30 patients with farmer's lung were tested with several strains of antigens. Reactions against two strains of T.vulgaris were shown in 46.7% and 66.7% of the patients group, which were significantly higher than in the controls, while the percentage of reactions against Faenia rectivirgula and A. fumigates were low in the patients group and had no significant difference with the controls. The results indicate that the main etiological agents were some strains of T. vulgaris. This finding was consistent with the microbiological study of sputum of the patients and moldy hay samples from the working area.

RESUMO

This article reports on an epidemiological survey of the presence of farmer's lung among 1054 farmers who grind moldy hay. The prevalence was 8.3-11.4%. The main pathogen was Thermoactinomyces vulgaris. The factors which influenced farmer's lung were the moldy state of the hay and the dust concentration. The epidemiological features of farmer's lung are analyzed and discussed and certain preventive measures are suggested.
